I declare I am an adult Elkhart Public Library cardholder, or an adult non-resident cardholder, in good standing. Further, I agree to release from liability Elkhart Public Library for any personal injury sustained during the use of library facilities. And further, I affirm the organization or group listed above is using library facilities for non-commercial purposes and is, through me, responsible for any infraction of the meeting room policy. This includes, but is not limited to, any damages to library property incurred during or in connection with the use of Elkhart Public Library’s meeting rooms.
